What this report is for
This is a light-vehicle condition report used for lease return, internal transfer, sale, return, and checkout. It can be downloaded as a CSV or PDF and filled in on screen. It includes dual signatures, noted damage, stations viewed conditions, VIN and miles, who is getting the vehicle, and who inspect the vehicle.
Tire numbers on this form match 49 CFR 570.9 and NHTSA TireWise: 2/32 inch remaining tread on light vehicles with GVWR 10,000 lb or less. Wheel cracks and fasteners are 570.10. Cabin and lamp looks follow ASE G1 and AAA multipoint practice. This is visible condition, not a paint-thickness map.
What it is not
- It is not a used-car buying walkthrough and not a CMV pre-trip.
- It is not a certificate of title, a registration, or the federal odometer disclosure. A transfer still needs 49 CFR 580.5 on the title or a separate 580 form. Lease return mileage from lessee to lessor is 580.7, a different disclosure.
- It is not the FTC Used Car Rule Buyers Guide. Dealers who must post that sticker still do so on the car, not on this PDF.
- It is not a state inspection sticker and not a shop lift PPI.
How to fill each block
State the purpose by picking checkout, return, sale, internal transfer, lease return, or other. Enter role/ name of both parties. Enter: Year, Make, Model, Color, Unit Number, Plate, VIN - Do Not enter any information from a dispatch note.
The odometer statement on this page are for information only. It does NOT meet 580.5. Odometer is whole miles, it does not include tenths.
Walk outside in daylight on level ground. Mark OK, Defect, or N/A on exterior, glass, tires, interior, and equipment. Note location and size of damage. There is no federal in-use chip-size Pass/Fail for light-vehicle windshields; state sticker and tint rules vary. Do not apply CMV windshield tables here.
What varies
If GVWR is at or below 10,000 lb then tires is 2/32 inch. If over 10,000 lb GVWR then front tire tread depth is 4/32 inch under 570.62. Look for a match on the axle. Check for any tread defects such as exposed cord, cuts, etc.
Operating Pressure comes from the door placard; do not look to side wall max for pressure. Keys/fobs should of been counted in the header. Even if body is clean, missing issued equipment remains a Defect on a return. Mark fleet-only rows N/A if they is sold privately.
Both parties will walk around the vehicle and note any damage with a dual sign-off; the receiving party may note a dispute on the vehicle. Check the photo box only if photographs was actually stored with this ticket.
After you finish
The template lacks rows for fleet damage grids, lease-end wear charts, dealer lot policies. That's because you go by the contract governing the handover. Rules differer than state to state with tint, windshield chips and inspections. This isn't a defect; it's just an inspection sticker on the glass.
The Fuel vs EV charge is a select on the header. When there is no fuel tank, you mark EV / N/A. Both parties get a copy. Each party should keep a copy (this PDF, a printout, or both). The answers are kept in the same browser tab as the form. When someone new visit they will see a blank form.
If transferring to another owner, fill out separate title, tax, odometer paper work. This report is not intended to transfer title.
Not a substitute for the OEM or a licensed inspection
The form is located above the article. Below the form is the print preview image and printable PDF version. It is not legal advice. It is not a replacement for a licensed inspection, a Buyers Guide from a dealer, odometer disclosure, or the OEM vehicle procedure.
